A coccyx fracture, also known as a broken tailbone or puzzle fracture, involves a break in the coccyx. This small bone sits at the very bottom of the spine, just below the sacrum, and represents the final segment of the vertebral column found in apes. In humans, it typically consists of three to five vertebrae that are fused together (though they can occasionally remain separate), measuring between 4 and 10 cm in length. The coccyx connects to the sacrum through a fibrocartilaginous joint called the sacrococcygeal symphysis, which permits only slight movement.

The most common causes of a coccyx fracture are falls directly onto the buttocks or the stress of childbirth. Several factors increase the risk, including low muscle mass, advanced age, osteoporosis, being female (due to the typically wider pelvis), and experiencing violence.

Symptoms include pain that worsens when sitting down, standing up from a chair, or during a bowel movement. There is often tenderness directly over the tailbone, along with possible nausea, bruising, or swelling in that area.

Diagnosis relies on a patient’s medical history and a physical exam that includes a rectal examination. X-rays, particularly dynamic (sitting and standing) views, are commonly used to help confirm the diagnosis and assess fracture displacement, though their role is sometimes debated in specific clinical contexts.

For most cases, treatment involves self-care at home rather than hospitalization, which is rarely needed. Patients are advised to sit on soft surfaces to reduce pressure on the coccyx. A donut or wedge cushion—essentially a cushion with a hole—can keep weight off the injured area. Pain relievers like ibuprofen are recommended, along with a high-fiber diet to soften stools and prevent constipation. Doctors typically do not attempt to realign a badly positioned fracture, as the surrounding muscles are strong enough to pull the bone back into place on their own. The bone is also difficult to immobilize due to the many muscles attached to it and its location.

Surgery is usually unnecessary. However, if pain persists after the fracture has healed and is severe enough to cause significant disability, a coccygectomy—surgical removal of the coccyx—may be considered.
